PEXiS 1 is the first module in a series of Master's level courses developed
by the VA Midwest Patient Safety Center exploring patient safety through
a human performance perspective. The course teaches strategies developed
in high-risk industries, accident analysis, and human factors methodology.
This module in the PEXiS curriculum introduces basic concepts about how
people may contribute to failures and how they create safety within an inherently
risky environment. You will learn about these concepts by analyzing multiple
low frequency/high consequence accidents, engaging in exercises about predictable
human behavior, and reading foundational material about human performance
issues in safety.
As you progress through the readings you will encounter practical exercises
that will both introduce, reinforce and demonstrate your grasp of this specific
topic: complex factors that lie behind the label 'human error'.
